I am a professional hairdresser with 20 years’ experience in the beauty industry. I specialise in colouring and am constantly expanding my knowledge of the latest hair colouring trends. I, the undersigned, confirm that I have been properly informed of all possible risks associated with hair colouring, chemical treatments, the products used and the possible results. 1. Allergic reactions I understand that any chemical or cosmetic product may cause an allergic reaction (e.g. skin irritation, a rash, itching, swelling or other symptoms), even if I have not experienced similar reactions in the past. I was offered an allergy test (patch test) prior to the procedure to help assess any potential sensitivity to dyes or other products used. ☑ The allergy test was carried out ☐ I declined the test of my own free will and accept full responsibility for any adverse effects 2. Variations in hair colour and understanding the result It has been explained to me that: The final hair colour result may differ from the chosen or expected colour due to the current condition of my hair, previous colouring, residual pigment, my natural hair colour and other individual factors. There may be variations in shade, particularly if the hair has already been coloured, bleached or damaged. 3. The final result may be achieved over several sessions I understand and agree that more than one treatment may be required to achieve the desired result (e.g. a lighter colour, an even tone or a neutral shade). The hairdresser has explained that colouring or lightening is a process, not always an instant result. I have also been informed of the recommended intervals between treatments to maintain the health of my hair. 4. Personal Responsibility I confirm that I have provided all relevant information regarding: Previous hair treatments (colouring, straightening, chemical perms, etc.) Any previous allergic reactions or sensitivities to cosmetics Any home remedies used that may have affected the condition of my hair (e.g. henna, natural dyes) I understand that the hairdresser or salon is not liable for any undesirable results or reactions if these occurred due to my providing incomplete or inaccurate information, or if I chose to proceed with the treatment at my own risk. --- CUSTOMER CONFIRMATION I understand and agree with the information provided above.
